RBHS Bruin Legacy Plaza Brick Order Form

In 2017 we will celebrate the construction of a new grandstand and concession stand. The Bruin Booster Club is offering you the opportunity to support Bruin athletics and be a permanent part of the stadium renovation by purchasing an engraved brick to include your own personal message.

Student Accident Insurance

Protect your student with student accident insurance. If you don’t have other insurance, this student accident insurance is vital. If you have other insurance, student accident insurance can help with deductibles and copays.

Physicians Clearance for Wrestler Body Fat Allowance

Any male wrestler whose body fat percentage, at the time of initial assessment, is below 7% must obtain a written physician’s clearance stating that the athlete is naturally at this sub-7% body fat level. In the case of a female wrestler, written physician’s clearance must be obtained for athletes who are sub-12% body fat. A physician’s clearance is for one season duration and expires March 1 of each school year.

Physical Evaluation Form

All CPS athletes must have a current physical form on file with their school. Physical forms are valid for a period of 12 months. Please fill this form out and take it with you to your health care provider. After the form is complete return it to the school your child attends.

MSHSAA Concussion Return to Play Form

If diagnosed with a concussion, an athlete must be cleared for progression to activity by an approved healthcare provider. This form must be filled out by your health care provider and returned to the school prior to participating in any activity following a concussion.

CPS Building Use Application
